The other pitchers were Mo-online, Snappie and Digitale Mentor. Mo-online is an online platform developed by the Philadelphia Zorg foundation. Set up for and by people with disabilities, it is an innovative, open platform — without login — where people with intellectual disabilities access reliable information in a safe and accessible way. It combines a strong online community with interactive learning and development opportunities, and connects various innovative applications that increase the target group's sense of control.
Snappie is an easy-to-use and secure AI tool that automatically generates support plans based on input from supervisors, clients and their network. This reduces administrative burden, increases clients' sense of control, and improves the quality and efficiency of support plans.
Digitale Mentor centralizes all the knowledge needed for intake, anamnesis and indication in the district-nursing process, making it easy to compile the entire care plan with minimal questions. All necessary knowledge is centrally and intelligently available digitally. Integration with the electronic care record (ECD) provides people with easy-to-read files, while the healthcare professional is guided throughout the entire process.
